Well, one would figure it would have to happen.  The statistics are against continual success.  Accidents, human error, natural interventions happen when least expected.

Recently, one of the featured Way to Cool artists nearly bought the ticket to heaven.  In his latest stunt for HBO, Jeb Corliss jumped from Table Mountain, South Africa.  Little did he know, the ledge of Table Mountain bit him, sending him into a spinning tumble.  Jeb managed to pull his chute to soften the blow.

2 broke legs and an injured neck later, Jeb is recuperating in a South African hositpal with, projected, months and possible years of rehabilitation and recovery.
